Doctors in Black Russia don’t shoot: they treat patients, issue medical records and prescriptions, and save injured players. You need a medical record to join any faction, so the whole server visits them.
Join the Hospital
Requirements:
- Account level 3;
- a medical record;
- a Class B driving licence.
You don’t need a military ID or a weapons licence. The Hospital has the lowest entry requirements after the Army. There are three hospitals: /gps → Important Places → City Hospital of Arzamas, Lytkarino or Nizhegorodsk. The 8-question interview is held at the desk opposite the entrance. One wrong answer means you fail; you can try again after 5 minutes. Find the answers in the HR department.
You can also apply to senior staff through the “Hospital” section of the server forum. Recruitment drives are announced through events. Once accepted, you start as an intern. The changing room is in the “Administration” wing, at the end of the corridor, in the staff lounge.
Compare pay by rank
Hourly pay, based on one server. The bonus for completing 5 tasks a day is ₽25,000–40,000. Ranks 8–10 (surgeon, department head and chief physician) perform senior staff duties. The top managers are the chief physician and department head.
See if the Hospital is right for you
Among factions that don’t require a weapons licence, the Hospital offers the best starting pay: interns earn ₽2,500 an hour, Army privates earn ₽1,950, and Media trainees earn ₽1,600. Government drivers earn ₽3,300, but they need a licence. The chief physician’s pay tops out at ₽16,980, almost as much as the Media director earns. The Hospital is a good fit if you mostly play in one city: there are three of them, so choose the one closest to your home.
The main advantage is the steady flow of people. You need a medical record to join any faction, and you can issue one from rank 3. The downside: no weapons or chases. You spend your shift in the building or responding to calls from injured players. I’d join if I wanted a quiet faction to have on in the background while I play.
Treat patients at charter prices
These prices come from the charter test. You can’t set your own:

Only doctors rank 3 or higher can issue medical records. Patients won’t ask a paramedic for one. The official guide says that a checkup at the “Get medical record” pickup costs ₽5,000 and takes 10 minutes. These are different prices, so check both on your server. Tasks and shifts
You can pick up tasks from the secretary or on the tablet, after completing the paperwork and tests. The guide lists four: “Medical Record King,” “Best Doctor,” “Draft Evasion” and “Rescue the Injured.” The rank table on the forum uses different names (“Medical Record Oversight,” “Secret Documents”), so the list may have changed. The two-vehicle fleet is in the inner courtyard. If you crash a service vehicle, you’ll be fined from your account.

The most common commands are /heal [ID] [price] to treat a player, /medhelp for treatment in a hospital bed, /gmedcard [ID] [price] to issue a medical record, and /ills to list injured players. Nearly all of these actions are also available in the radial menu: long-press a player → “Service.”
Follow the Hospital charter
- You can’t refuse treatment to anyone, including members of gangs.
- Setting your own prices breaks the charter.
- Carrying a weapon in your work uniform means 30 minutes in jail. Self-defense is the exception, but you must have a video recording of it.
- You can wear glasses and bags with your uniform. You can only go AFK (away from your device) in the changing room.
- Don’t go to the casino or container events in uniform. Don’t use a service vehicle for personal trips.
- Use the [ЦБ] tag in the department radio, not [Больница].
Leave through the tablet or the chief physician
Tablet → Factions → Roster → your nickname → “Quit.” Without the “Determination” upgrade or Silver VIP or higher, the button won’t work. In that case, ask the chief physician or department head to dismiss you. Your rank will reset.
